The flushes are sometimes in vials with yellow labels, which is helpful. Yes, we all do make mistakes - I'm a nurse, I'm well aware of the working conditions. However, I can't have much sympathy for someone who says, "Oops, I didn't look at the medication before I gave it."

I used to be a pharmacy tech I know what those vials look like. The company that manufatures them makes them all the same size and different doses are printed in similar colors. The vials are teeny, tiny and the print on them is tensy and there is a lot of it. Easy to mistake them. Overdoses of Heparin have been a problem for many, many years and still the company makes them in those damn vials with the same print. I wonder how many people would be alive today if the pharmaceutical companies just packaged things differently. Nurses, doctors, techs all makes mistakes. They are not super human. So they cannot take all of the blame.

I'm glad that the FDA made a formal statement.

Yet it take 30 seconds for a nurse to stop and look at a drug label before drawing up the medication. Heparin is a known dangerous drug! In fact, if that hospital uses the popular med carts that most hospitals use, the screen says "This is a high alert medication!" when you select it. I understand how busy it can get, but really now, there's no excuse.
